Physician arrangements without a complete signed writing
Compensation arrangements with referring physicians generally have to sit inside a Stark Law exception, and those exceptions require a signed writing before compensation is paid. An expired, unsigned, or unlocatable agreement that is still being paid is the exposure the rule exists to prevent. Nothing here is legal advice, and the register cannot see your payment systems — it holds no invoice or payroll data. This is a list of documents that cannot currently be produced, which is the half a contract register can answer. Whether money is still moving against them is the question to take to accounts payable.
Term ended, no close-out recorded
The term has passed and nobody marked the agreement renewed, terminated or sunset, so it is sitting in limbo. Whether money is still moving against it is NOT something this register can see — that needs a feed from accounts payable, and until one exists the only honest statement is that the record was never closed. Where somebody has checked and written it down, it appears in the summary as their note, not as a system finding.
Renewal cannot be assessed
The notice period or the term dates were never captured, so there is no deadline to compute. These are the ones that look fine on a spreadsheet precisely because the cell is empty.
Numbers that do not follow the convention
The register files these under a group, but their number says something else — or nothing at all. Reports are unaffected, because they aggregate on the recorded group rather than by reading the number. It matters because a number that disagrees with the record is what makes people stop trusting the register and go back to asking around. These are almost always contracts that predate the convention.
No owner assigned
An alert with no addressee is not a control. Every renewal decision needs a named person before the notice window opens.
